HANDS Ongoing Home Visitor.
The Montgomery County Health Department is accepting applications for a full-time HANDS Ongoing Home Visitor.
Job Description: H.A.N.D.S. is a program that provides support to new and expecting parents. from the prenatal period through the first two years of the child’s life. H.A.N.D.S. helps new parents to promote optimal growth and development of their child by providing support in numerous ways.
Job responsibilities include, but are not limited to:
- Do weekly supervision with HANDS Supervisor (1.5 hours).
- Complete all HANDS trainings per HANDS policy timelines (or before).
- Accept families onto caseload until Employee has a minimum of 18-20 families.
- Maintain a caseload that enables a minimum of 60 visits a month to be completed.
- Complete all documentation (forms and home visit logs) within 24 hours of the home visit completion.
- Keep a calendar of all completed visits, attempted visits, and canceled visits.
- Provide HANDS Program Manager with calendar and end-of-month reports on the first day of the following month.
- Submit a time sheet every two weeks with travel and cell phone allowance.
- Required to complete all annual health department training courses and follow the health department’s policies and procedures.
- Other duties as assigned.
Minimum Education, Training or Experience: High School Diploma but A Bachelor’s degree in Social Work or an Associate’s Degree in Nursing is preferred.
Salary: (Negotiable depending on education and experience.)

Health Educator

Job Description: Job responsibilities include, but are not limited to:
- Provide health and wellness education to diverse populations across the lifespan.
- Plan, develop, and implement programs designed to address community needs.
- Facilitate classes according to organizational and program guidelines (ex. Smoking Cessation, Diabetes, Sexual Health)
- Develop, prepare, and coordinate grant applications and activities to obtain funding for health education programs and related work.

Reportable Disease /Employee Health/Clinic Nurse
The Montgomery County Health Department is accepting applications for a Full-time Registered Nurse Position. This position will be for a Reportable Disease /Employee Health/Clinic Nurse
General Duties:
- This individual will be responsible for infectious disease monitoring for Montgomery County which includes surveillance, conducting disease investigations, and assisting with management of community outbreaks.
- They will also be responsible for all functions of the employee health program which will include ensuring employees are compliant with TB assessments/skin testing, fit testing, and vaccinations. They will also assist with updating internal employee health policies and manuals to ensure compliance with OSHA such as Respiratory and Bloodborne Pathogen Plans and SDS sheets/manual.
- They will participate as an integral team member, collaborating with various departments and colleagues, including the Health Department Director, the Director of Nursing, Human Resources, the Environmental Director, and others as needed to advocate for employee health and safety. They will be responsible for the management of potential blood exposures, employee illnesses, and disease outbreaks.
- They will provide case management and treatment as indicated for clients with TB Infection and active TB Disease which can include direct observation of therapy, home visits, tracking and conducting investigations, providing education, and treating contacts of active tuberculosis patients. They will work closely with the Director of Nursing and contracted Infectious Disease Provider in this role.
- They will conduct COVID-19 surveillance for Montgomery County and assist with outbreaks when needed. Duties may include providing education and/or information to staff, physicians, clients, and the public on current CDC guidelines pertaining to COVID-19 as well as local disease trends.
- They will collaborate closely with and report directly to the Director of Nursing.
- Duties will also include providing clinic services including but not limited to WIC, Immunizations, and STI testing, and treatment as needed. They may also include working special events in disease prevention efforts such as vaccination events and outreach.
Minimum Education, Training, or Experience: Nursing License. ADN or BSN Nursing is acceptable.
Starting Salary: $20.00-$25.00 per hour, negotiable based on experience
Benefits: This is a full-time contract position. The work week will be 37.5 hours per week.
