Am I at risk for Type 2 Diabetes?
- Are you 45 years or old?
- Are you over weight?
- Do you have a family history of Type 2 Diabetes?
- Are you physically active fewer than three times per week?
- Did you have Gestational or give birth to a baby more than 9 pounds?

The Montgomery County Diabetes Prevention Program is a proven way to prevent diabetes in people who have Pre-diabetes or are at risk for Type 2 Diabetes by making small lifestyle changes.
A lifestyle coach will help you develop skills to improve food choices, increase your physical activity for weight loss, and encourage healthy lifestyle changes.
Our program meets in a small group for just one hour every week for 16 weeks and then monthly.
