The Healthy Living with Diabetes Program mission is to empower individuals with diabetes to make knowledgeable, responsible choices and behavior changes regarding self-management of their diabetes.

Individuals may schedule a Medical Nutrition Therapy (MNT) appointment with our Registered Dietician and Licensed Diabetes Educator. To schedule an appointment, please call 859-498-3808 and press 0.

The Montgomery County Diabetes Prevention Program is a proven way to prevent diabetes in people who have Pre-diabetes or are at risk for Type 2 Diabetes by making small lifestyle changes.
A lifestyle coach will help you develop skills to improve food choices, increase your physical activity for weight loss, and encourage healthy lifestyle changes.
Our program meets in a small group for just one hour every week for 16 weeks and then monthly.
